A theory of change for community-initiated mental health care in the United States

Mental health service delivery needs radical reimagination in the United States where unmet needs for care remain large and most metrics on the burden of mental health problems have worsened, despite significant numbers of mental health professionals, spending on service provision and research. The COVID-19 pandemic has exacerbated the need for mental health care. One path to a radical reimagination is “Community Initiated Care (CIC)” which equips and empowers communities to address by providing brief psychosocial interventions by people in community settings. We co-developed a theory of change (ToC) for CIC with 24 stakeholders including representatives from community-based, advocacy, philanthropic and faith-based organizations to understand how CIC could be developed and adapted for specific contexts. We present a ToC which describes ways in which the CIC initiative can promote and strengthen mental health in communities in the United States with respect to community organization and leadership; community care and inclusion and normalizing mental health. We propose 10 strategies as part of CIC and propose a way forward for implementation and evaluation. This CIC model is a local, tailored approach which can expand the role of community members to strengthen our response to mental health needs in the United States.

Linguliform brachiopods across a Cambrian–Ordovician (Furongian, Early Ordovician) biomere boundary: the Sunwaptan–Skullrockian North American Stage boundary in the Wilberns and Tanyard formations of central Texas

The Cambrian-Ordovician Diversity Plateau, between the Cambrian Explosion and the Ordovician Radiation, is punctuated by a series of well-documented Laurentian trilobite extinction events. These events define the bounding surfaces of trilobite ‘biomeres’ that correspond to North American stages, including those of the Sunwaptan and Skullrockian. Trilobites show a consistent pattern of recovery across these boundaries, and commonly each extinction and replacement of taxa is interpreted as a single event as changing environmental conditions spurred shoreward migration of shelf or oceanic faunas that displaced established cratonic faunas. Linguliform brachiopods are also abundant in strata of this interval, and we investigate their stratigraphic distribution across the Sunwaptan–Skullrockian Stage boundary in Texas through high-resolution stratigraphic sampling of subtidal sediments. We document complete genus- and species-level turnover of the linguliform brachiopod fauna coincident with trilobite extinction events, suggesting that these brachiopods were affected by the same factors that affected trilobites. The Skullrockian replacement fauna was cosmopolitan, with ties to Gondwana and Kazakhstan and to the Laurentian shelf environment. The timing of appearances of taxa suggests that the faunal migration onto the Laurentian shelf came from elsewhere during a transgression. The disappearance of the Sunwaptan fauna and the arrival of the Skullrockian fauna are distinct events. We suggest that ‘biomere’ events may be complex, and the cause of the extinction is not necessarily the same event that facilitates the appearance of a replacement fauna. We describe one new species, Schizambon langei.

Crinoid columnals are major faunal components of interbedded shales and carbonates of the Upper Ordovician Kope to Fairview formations (Edenian-Maysvillian) of the Cincinnati Arch region. Six species can be identified on the basis of distinctive morphological characters of the columnals. Crinoid distribution was plotted from point-counted carbonate samples taken through a 68-m thick composite section of the Kope to Fairview formations in Campbell County, Kentucky. This section spans a shallowing-upward, third-order depositional sequence (C1), part of C2, and the Edenian-Maysvillian Stage boundary. The slender cladid crinoid Merocrinus occurs in the lowermost Kope below the base of this section. The slender disparids Cincinnaticrinus and Ectenocrinus occur throughout the section but are most abundant in the lower 25 m where the shale percentage is 60–90 percent. The larger, more robust disparid Iocrinus appears within the carbonate-rich Grand Avenue member of the Kope at 40–50 m, and the large, plated camerate Glyptocrinus first appears just above the Grand Avenue and becomes the dominant crinoid above the C1–C2 sequence boundary that lies just above the Kope-Fairview contact. The largest and most robust crinoid in this sequence, Anomalocrinus, occurs at the top of the Grand Avenue Member. Siliciclastic ratio and biofacies composition indicate that the occurrence of larger, more robust crinoid taxa is correlated with shallowing depth. Crinoid trophic niche differentiation is also correlated with decreasing depth and the concomitant increase in water movement caused by waves and currents. The deeper water disparids have a nonpinnulate filtration fan with low branch density and wider ambulacral grooves. The shallower water camerate Glyptocrinus has a pinnulate filtration fan with high branch density and narrower ambulacral grooves. These relationships are consistent with the predictions of aerosol filtration theory.
